USCIS Immigration Examination Information

What is USCIS examination

Our physician is a licensed U.S. Civil Surgeon who has been authorized by the Bureau of Citizenship and Immigration Services to conduct Immigration Physical Examinations for the Department of Homeland Security.
This examination consists of:

  • A detailed review of your medical history and immunization records

  • A physical and mental evaluation

  • Tests for various diseases and illnesses

  • Completion of the I-693 form

WHAT TO EXPECT

Two appointments in our office for adults, one for children

  • 1st appointment includes review of medical history, the physical examination and arrangement for vaccinations and/or testing (if needed).

  • 2nd appointment includes review of lab results or X-ray (if required due to abnormal blood test), additional vaccinations if required and completion of I-693 forms.

COST

With Insurance: 

  • Office visits, labs and any vaccination is initially billed to insurance. You are responsible for co-pay, deductible or any services not covered by insurance. Please keep in mind that not all insurance companies cover this type of exam. So check with your insurance about the coverage.   

  • There is additional $75.00 fee for completion of I-693 forms and associated administrative work. A $25.00 non-refundable deposit is required to book an appointment at the time of scheduling. The deposit will be applied to the $75.00 administrative fee and the remaining balance will be collected during check in. 
     

Without Insurance: 

  • $400 per person for office visits and completion of I-693 forms. If any lab work or vaccinations are required, information on associated costs will be provided.

WHAT TO BRING

  • Valid government issued picture ID such as a driver license or passport

  • Vaccination and other relevant medical records

  • If attainable, please bring a list of any immunization or vaccination records. If your records are from your home country, and not in English, it will benefit you to have our physician review them.

  • If you have ever been diagnosed with TB, you must present a written certification, signed by your doctor, proving that you were adequately treated. The certificate must include dates and types of medications you took. If you have ever had an abnormal chest X-ray, borrow the last X-ray films taken and bring them to your medical exam.

  • If you have had syphilis please bring a written certificate, signed by a physician or public health official, proving that you were adequately treated. If you have ever had a positive VDRL or other blood test for syphilis, and were not treated, you must bring a written explanation signed by your physician.

  • Bring a list of any medication you are taking. If you are being treated for a chronic medical condition or taking medication on a regular basis, be ready to explain your condition(s).

AFTER THE EXAM

After the immigration physical examination is completed, our physician will certify the I-693 form. This paperwork will be given to you in a sealed envelope. DO NOT OPEN THE SEALED ENVELOPE. Turn in the envelope with your immigration application. A copy of the enclosed forms will also be provided for you to keep. The examination will only be valid up to a year after the immigration exam date.
